Where is perineal

It sits below the pelvic diaphragm and contains openings and sphincter functions of the urethra, vagina, and anus. Between the vaginal opening and the anus is the Perineal body, which serves as a center attachment point for superficial and deep pelvic floor layers. During delivery, these superficial and deep tissues need to have the flexibility to stretch and allow the baby to pass through the birth canal.

Some women during birthing may experience trauma to this area such as tears or episiotomies. As well, may experience pain in this area for weeks postpartum. Most women experience some trauma to the perineum when giving birth to their first child. Some will experience more severe tearing that extends beyond the vagina, deep into either the adjacent muscle or the anus. Tears can be painful and make it difficult to walk, use the restroom, or perform other daily functions.

This pain can last for several weeks. Tears can affect the muscles beneath the perineum. More severe tears can cause ongoing muscle problems, which can be very painful and may affect the function of the urinary bladder or even that of the anal sphincter.

Some women also experience pain during sex. Regular sitz baths, cold packs, and numbing sprays may help treat the pain.

A woman should see a doctor if the tear occurs with signs of infection, such as:. After the tear heals, some women use pelvic floor physical therapy to manage chronic pain. An episiotomy is a procedure in which doctors make a small cut into the perineum to expand the vaginal opening during childbirth.

This procedure is still fairly common during childbirth. However, an increasing number of experts are recommending against the use of routine episiotomies, as some research suggests that they might increase the risk of incontinence and pelvic pain. The pain resulting from an episiotomy is similar to that of a perineal tear. The severity of the symptoms and the healing time will vary among women, depending on the type of tear or episiotomy.

Sitz baths, numbing sprays, and over-the-counter pain relievers can provide short term relief. Pelvic floor physical therapy is useful for managing symptoms in the long term.

In some cases, the episiotomy can injure surrounding areas, and it may make surgery necessary. Both males and females can experience other perineal injuries. Trauma, such as that from a fall or an assault, can damage the muscles or skin of the perineum. Some health conditions, including diabetes , can cause nerve damage in the perineum.

The right home treatment will depend on the injury. Nerve pain will often require treatment from a doctor, but some scratches or cuts will improve with sitz baths and pain relieving medication.

Regardless of the cause, it is always best to see a doctor about perineal injuries. The pelvic floor muscles sit at the bottom of the pelvis and support pelvic organs, such as the urinary bladder. Pelvic floor dysfunction can occur when the muscles become too tight, causing pain and tension. Perineum The perineum is an area located at the lowermost aspect of the pelvis just inferior to its floor and between the thighs. It is diamond-shaped and can be divided by an imaginary line drawn between the two ischial tuberosities , into an anterior urogenital triangle and a posterior anal triangle.

Blood supply to the Perineum is from the Pudendal nerves S2 to S4 and the pudendal vessels. Lymphatic drainage is to the superficial inguinal, deep inguinal and the internal iliac nodes. This triangle is located in the anterior part of the perineum. Its base is formed by an imaginary line connecting the two ischial tuberosities and is bounded by the pubic symphysis anteriorly and the ischiopubic rami on either side.

It is composed of the urethra and the external genitalia. The urogenital triangle consists of several layers of fascia. From superficial to deep the layers of the fascia are:.

Skin : it is the epithelial surface onto which the urethral and the vaginal orifices open. Superficial Perineal fascia : this fascial layer is continuous with the superficial layer of the fascia of the abdominal wall.

It consists of two layers — a superficial, fatty layer which forms the labia majora and mons pubis in females and a deep layer fascia of Colles. Superficial Perineal pouch : is a potential space located between the deep perineal fascia and the perineal membrane.

Perineal Membrane : The perineal membrane is a dense fascial layer which covers a major part of the urogenital triangle. It provides support to the pelvic diaphragm superiorly and anchors the external genitalia inferiorly. The urethra and the vagina pass through the perineal membrane to open onto the surface of the skin. Deep perineal pouch : this is a potential space between the perineal membrane and the pelvic floor.

The external urethral sphincter, part of the urethra and in males, the bulbourethral glands and the transverse perineal muscles are contained within this pouch.

The perineal body is a midline structure formed by dense connective tissue. It anchors the perineal membrane as well as provides attachment to several perineal muscles:. It is located between the penile bulb and the anus in males while in females it lies between the vagina and external anal sphincter.

It provides support to the posterior vaginal wall and prevents vault prolapse in females. The perineal structures in males and females develop from identical structures. The male penis is identical structurally to the clitoris in females but is wider. The clitoris is composed of the corpora cavernosa, is covered by the prepuce and ends in a glans similar to the penis in males.

The corpus spongiosum in males is equivalent to the vestibular bulbs in the females. The perineal body is clearly visible on transperineal ultrasound in healthy women 3. The perineal body can be easily identified on thin slice MRI in healthy women and its internal structure is appreciable on optimized sequences 4.

Injury to it during childbirth may weaken the pelvic floor and contribute to prolapse of the vagina and uterus.
